Our Mental Health practitioner supports individuals aged 18+ with severe mental illness.
Your practice will assess if our practitioner can help and if a telephone call or face 2 face appointment is appropriate.

Our Social Prescribers are able to see patients who are 13 years plus.
They provide holistic patient assessments through motivational interviewing and guided conversations around their emphasis on "What Matters to Me".

Our MSK First Contact Practitioners
assess patients with soft tissue, muscle, and joint pain and decide on the most appropriate management pathway. Our practitoners can refer patients to hospital services where necessary, order tests and write prescriptions.

Please note you may be asked to attend other surgeries.
Our Physician Associates work alongside GP’s and are fully supported allowing them to see patients, order investigations and manage conditions.

Our Paramedics visit housebound patients at home, when requested by a GP.
Our paramedics are skilled to deal with many conditions.
We cannot give exact times for home visits.

Our Ageing Well Team works closely with patients to ensure they have the correct support and care plans in place.
Home Visits are usually undertaken between 10am – 2pm but you may receive a telephone call outside of this time.
