Ageing Well Team

Our Ageing Well Nurse and Paramedics work closely with patients to ensure they have the correct support and care plans in place. 

Your practice may refer you to the Ageing Well Team if they think you will benefit from their input. 

Some patients receive letters inviting them to an Enhanced Care Review – you may be asked to let your practice know if you would like to take up this offer. 

Home Visits are usually undertaken between 10am – 2pm but you may receive a telephone call outside of this time. 

Our Ageing Well Care Coordinator provides administrative support to the Ageing Well Nurse and Ageing Well Programme.  You may receive a phone call from the coordinator to arrange a day for the nurse to visit.

The Dorset Care Plan (DCP)

Our Ageing Well Team aim to offer our most vulnerable patients a Dorset Care Plan and to update this at least annually. 

DCP

You and your carers will be asked to contribute to your Dorset Care Plan. For example, you will be asked for contact details for your next of kin and carers and if you need help for your day to day living. You will also be asked if you have preferences for hospital admission in certain circumstances. This will help build up a picture of what you want to happen if you become unwell.

Telephone calls

Please bear in mind that we could be at any of our practices when we call you so you may not recognise the phone number calling. We will try you again if no answer. If you miss an expected call and want to call back, please ring your registered practice. 

Summary Care Record (SCR)

Most patients have a core SCR showing basic information such as medications and allergies. You will also be asked if you give permission to add additional information to your SCR. This will enable health professionals who work for other NHS services to view extra information such as the information from your DCP,  serious health conditions and treatment preferences. There are strict rules about who can view this information.

You don't have to wait for your DCP review to opt into adding additional information to your SCR.  You may have already opted in when you registered at your practice or you can use the link below to download a consent form - please return completed forms to your practice.
